Huge congratulations are in order for the CHI at Crumlin Complex Care Coordination team, alongside 22q Ireland, who have today won an HSE Excellence Award for their project:
Complex Care Coordination: A Transferable Model of Care for Rare Disease - CHI & 22q Ireland
The HSE Excellence Awards are an opportunity to showcase and celebrate examples of the great work that happens every day across our health service. They aim to encourage and inspire health service staff to develop and improve care and services for patients.
Dr Suzanne Kelleher, Consultant Paediatrician, and Wesley Mulcahy, Complex Care Coordinator, worked with Anne Lawlor, Chairperson of 22q Ireland, on their project, which has made a huge difference to the running of the clinic for patients with a rare disease called 22q11 Deletion Syndrome.
Instead of coming to CHI at Crumlin for multiple different appointments, these patients’ care can now be coordinated on the same day, or can be coordinated in the community. Wesley Mulcahy, Complex Care Coordinator, is the single point of contact for questions that come up in relation to the ongoing care of a young person with 22q11, all of which reduces the stress and burden on parents and extended families. The team won the HSE Excellence Award for demonstrating that parental liaison and putting young people at the centre of their healthcare decision is having positive outcomes for people living with rare disease.
