Data protection

In May 2021, the HSE cyber-attack resulted in a data breach. Children’s Health Ireland was impacted by this data breach as some ICT systems are shared with the HSE.

Request your documents by post

If you have received a letter from CHI about the HSE Cyber-Attack, you can request copies of your breached documents.

Request your documents by post

You can request your breached documents by post. This is the only way to request these documents. You can no longer do this online.

What you need to do

To make a request you will need:

  • your PIN number - you can find this on the letter we sent you,
  • to print out and complete the Data Access Request Form (PDF, 1.4 MB, 1 page),
  • to photocopy your current and in-date passport ID page, driving licence or public services card (photocopy both sides of your driver's licence or public services card).

How long your documents will take?

It can take up to 1 month for the documents to be ready from the date that we receive your completed form and photocopy of your ID and/or evidence of legal guardianship.

In some cases, it may take up to 2 months longer. We will let you know if this happens and explain why.

When we send you your documents we will include details of how to contact us. This is if you want to speak to us about the information you receive.

If you need to change your postal address

To update your postal address, you will need:

  • your PIN number - you can find this on the letter we sent you,
  • to print out and complete the Change of Address Form (PDF, 1.4 MB, 1 page),
  • to photocopy your passport, driving licence or public services card – if using your driver’s licence or public services card, photocopy both sides,
  • proof of your address such as a utility bill or bank statement dated within the past 3 months.
