Access to the New Form

• The new Genomic Testing Request Form is attached to this communication
• It is available from the CHI website: https://www.childrenshealthireland.ie/list-of-services/clinical-genetics/#our-services
• Alternatively, you can request a copy by contacting the laboratory directly at genetics.lab@childrenshealthireland.ie

Transition Period

• Both the new and old version of the form will be accepted for a period of 6 months (until September 2025)
• After this period only the new version of the form will be accepted

Improvements

• Improved layout and modified consent section for easier completion
• Page 2 provides revised sample acceptance guidelines to allow for ease of completion and efficiencies
• Informed consent has been replaced by a consent statement that must be signed by the referring clinician.
• A checkbox is included for consent to DNA storage, to be ticked by the referring clinician if consent for storage was provided.
• There is no longer a requirement for patient/guardian signature

Why Change?

• Edits are based on User feedback
• The consent section of the request form had been highlighted as causing the most frustration amongst Users
• In 2024, the draft version was circulated to over thirty key Users and feedback was overwhelmingly positive.
