Genomic Testing Request Form DCG
We are pleased to inform you that the Department of Clinical Genetics Laboratory has updated its Genomic Testing Request Form, and the new version is now in use.

To ensure a smooth transition, please find the updated form attached. We kindly ask that all copies of the previous version are discarded and replaced with the new form.
Access to the New Form
- The new Genomic Testing Request Form is attached to this communication

- Alternatively, you can request a copy by contacting the laboratory directly at genetics.lab@childrenshealthireland.ie
Transition Period
- Both the new and old version of the form will be accepted for a period of 6 months (until September 2025)
- After this period only the new version of the form will be accepted
Improvements
- Improved layout and modified consent section for easier completion
- Page 2 provides revised sample acceptance guidelines to allow for ease of completion and efficiencies
- Informed consent has been replaced by a consent statement that must be signed by the referring clinician.
- A checkbox is included for consent to DNA storage, to be ticked by the referring clinician if consent for storage was provided.
- There is no longer a requirement for patient/guardian signature
Why Change?
- Edits are based on User feedback
- The consent section of the request form had been highlighted as causing the most frustration amongst Users
- In 2024, the draft version was circulated to over thirty key Users and feedback was overwhelmingly positive.
