Healthcare record and personal data requests

You have the right to request access to a copy of your/or your child’s personal data and find out what information is held about you/or your child in CHI on computer and manual records.

You can also make a request for data to CHI under the legislative provisions of General Data Protection Regulations (GDPR) 2016 / Data Protection Act 1988-2018 or the Freedom of Information (FOI) Act 2014
